Submit All Required Documentation
- Birth Certificate
- Recent photo
- Social Security or Passport
- SUFS Award Letter (if using scholarship funds)
- Medical Forms (3040, parent side completed and signed and 680)
- Report cards and testing scores for the current and previous year.
All forms may be emailed to info@lwcalions.com or dropped off at the school office.

Academic Screening & Family Interview
- Once your application is submitted, you will receive a phone call to set up the academic screening and the family interview.
Acceptance
- You will receive an email letting you know if your child is accepted into LWCA.
- Pay the non-refundable registration fee to secure your child's space.
