Living Water Christian Academy - Preview Day Liability Waiver
In consideration of being allowed to participate in the Preview Day event at Living Water Christian Academy (LWCA), I, the undersigned parent or legal guardian, hereby acknowledge and agree to the following terms and conditions:
1. Assumption of Risks: I understand that participation in the Preview Day involves certain risks and dangers, including but not limited to accidents, injuries, and property damage. I voluntarily assume all such risks on behalf of my child.
2. Health and Medical Conditions: I certify that my child is in good health and has no physical or medical conditions that would prevent their safe participation in the event. I agree to inform LWCA of any health or medical conditions that may affect my child's participation.
3. Emergency Medical Treatment: In the event of illness, injury, or medical emergency, I authorize LWCA and its representatives to obtain necessary medical treatment for my child. I understand that LWCA will make reasonable efforts to contact me in case of an emergency.
4. Photography and Publicity: I grant LWCA the right to take photographs and/or videos of my child during the Preview Day event. I authorize LWCA to use and publish these materials for promotional and publicity purposes without compensation.
5. Behavior Expectations:*I understand that my child is expected to follow all instructions and rules set by LWCA staff during the event. Failure to comply may result in my child being removed from the event.
6. Release of Liability: I release and discharge LWCA, its staff, employees, volunteers, and agents from any and all claims, liabilities, demands, or causes of action that my child may have for injuries, damages, or losses sustained during or as a result of the Preview Day event.
7. Indemnification: I agree to indemnify and hold LWCA harmless from any claims or actions brought against LWCA as a result of my child's participation in the event.
I have read this waiver and fully understand its terms. I acknowledge that I am signing this waiver voluntarily and with full knowledge of its significance.
